Canceled appointments

If you are unable to keep your appointment, please notify us as soon as possible. We will be glad to reschedule your appointment to a more favorable time. We greatly appreciate your consideration in giving at least two business days notice for cancellations and look forward to seeing you at your new appointment time.

New patient forms

If you have made your initial appointment, you can preregister by clicking on the link below to download our New Patient Forms. Please print and complete the forms prior to your arrival. This will help to speed up the new patient registration process.


We work with most private insurance plans. If you have orthodontic insurance coverage, please bring documentation with you so that we can rapidly determine your benefit. For your convenience, we can usually accept the insurance benefit directly from the provider and coordinate all necessary paperwork.

  • CareCredit Financing: We offer easy and affordable monthly payment options—often with no interest.
  • Flexible Spending Accounts: If you work for a company that provides a flexible spending account, or a “flex plan,” we will explain the mechanism for saving up to 40 percent of your treatment cost by paying with pre-tax dollars.
